Sidebar Form is a friendly plugin generating a contact form on your WordPress. After activating the plugin you can add the widget called "Sidebar Form" to your sidebar or insert the shortcode [SIDEBAR-FORM] to one of your page or post. You will need to make some simple modifications in the preferences such as the email address you want to receive the contact informations.
If you are looking to implement a contact form on the page, please use the Contact Us From Plugin instead.
The plugin embed some CSS but if you feel like using your own CSS you can add it on the plugin preferences (under the Style section). You can also customize some fields, use captcha… everything is included!
